Go to Amazon Music for Artists
Click Get Started, then click Get Started again.
Sign in to your Amazon account (TIP: if you have multiple Amazon accounts use the account you used to claim your artist profile, otherwise use the account profile you intend to claim your artist profile with)
Once signed in, click User Settings > Report Issue.
Complete the form and click submit.
Please note that it may take up to 4 weeks for changes to appear on Amazon.com after submission.
